In order to create a safe and effective learning environment for your child, it is important that we, as educators, have a full understanding of each child’s special needs that may in any way impact their experience in our Religious School.
When we are provided with IEPs or similar documents, we are able to carry learning modifications through from your child’s secular or day school into our classrooms and programs, for your child’s benefit. When we are aware of allergies, medications, and other medical issues, we can take appropriate steps to ensure your child’s health and safety while in our school.
Please be assured that the information in this form will be shared only with the Cantor/Director of Education and your child’s teacher unless parental permission is otherwise expressly granted.
If you answered “yes” to any of these questions, the Cantor/Director of Education will contact you to discuss further information, action plans, and any questions or concerns you may have.

Examples of bullying may include, but are not limited to, the following:

Every student and his or her parent(s) must return a signed Brit Kehillah to the Cantor/Director of Education each school year, before the student may attend the Religious School of Congregation B’nai Tikvah for that year.
