Parental/Guardian Permission For Use of Photographs

I grant the Jewish Federation in the Heart of New Jersey (the "Federation") and Congregation B'nai Tikvah permission to take photographs of me, my children, and my property in connection with the above-identified event. I agree that the Federation may use such photographs of me and my children with or without our names and for any lawful purpose, including, for example, purposes such as publicity, illustration, advertising, and web content.

Please note that this is a PJ Library program, and that the information provided above will be shared with PJ Library in the Heart of NJ and the Jewish Federation.
